Twinning agreement with 4 Dakar sites and 4 French Sites. Twinning agreement with 4 district hospitals in the suburbs of Dakar and a French site. Support is also extended to an association network and care structures (23 partners).

Twinning agreement signed between a Senegalese Hospital and a French hospital. Three associations that are members of an association network (including hospitals) complete the system.

Chronology

Senegal was the first country in the ESTHER Initiative to have signed the Administrative Arrangement between the French Ministry of Health and the Senegalese Ministry of Health on April 12, 2002. This Arrangement was signed by Professor Awa C. Seck and Doctor Bernard Kouchner. All of the hospital agreements between partner hospitals were signed at the same time. Certain twinnings are rooted in pre-existing partnerships. From the end of 2006, ESTHER support has extended to actions by Senegalese civil society organizations with a view to promoting comprehensive and sustainable care. From that time, the association network (treatment platform) has been active in Dakar and Saint Louis. In 2007, some partnerships were refocused and experienced a new dynamism: CHU Fann/APHP St Antoine, Albert Royer National Children's Hospital/APHP Robert Debré. There was a slowdown in activities in 2008 owing to budget reductions.

In 2008, the ESTHER Initiative contributed to the ARV therapy of 2,700 regularly monitored patients including 194 children.

MAJOR ACTIVITIES IN 2008


§ Treatment and care of patients receiving ARV therapy and with Opportunistic Infections

(continuation of the 2007 project)

- Technical and financial support for clinical diagnosis and treatment of OI, in particular at CHU Fann (CTA, Department of Pulmonology, Department of STD), installation of a video endoscopy unit in the Department of Pulmonology.

- Training in the diagnosis and treatment of opportunistic infections with the major support of the parasitology, bacteriology and virology laboratories.

- Training in hospital hygiene.

- Internship in pulmonology at APHP St Antoine.

§ PMTCT & Paediatrics (continuation of the 2007 project)

- At the 4 Health Centres of Pikine, Rufisque, Mbao and Guediawaye: supplies of milk for infants and reagents.

- Support for paediatric treatment at Albert Royer National Children's Hospital: treatment, support for capacities in diagnostics, biological monitoring and psychosocial support.

- Training in adherence monitoring for children for 2 midwives and 1 psychologist.

- Training in HIV treatment and care for care providers in Dakar paediatric departments.

§ Reinforcement of biological diagnosis (continuation of the 2007 project)

- Continuous training in good laboratory practices (parasitology internships at APHP St Antoine).

- Support in reagents and equipment maintenance.

- Financial support for patient biological work-ups.

§ Psychosocial support for patients (continuation of the 2007 project)

- Training for 21 mediators in psychosocial counselling in partnership with Sida Info Service.

- Financial support for reception and lodging centres.

- Nutritional education.

- Support by a psychologist.

- Home visits.

- Consultations and therapeutic education clubs.

- Hospital duty.

- Support of orphans (access to schooling, food kits).

- Training for psychosocial support in France for a social worker.

§ Monitoring-Evaluation

- Training in the use of ESOPE software for 34 care providers at CHU Fann.


2008 IN NUMBERS

5,500 monitored PLWHA benefitted from ESTHER support (including 2,700 receiving ARV therapy including 1,000 on OI medicines).

 6 care providers on internships in France in specialised departments.

 1 social worker received a University Certificate in Ouagadougou.

 20 peer educators trained by Sida Info Services in the psychosocial support of patients.

200 PLWHA benefitted from nutritional monitoring, support for IGA projects and schooling for 300 VOC.
